Is this caused by Crohn's

The bones in my toes and fingers crack very easily and when I bend my wrist back it feels like the bones are grinding and its very painful. Could this be an extra intestinal manifestation of Crohns?

Could be arthritis related which is. An x-ray will show if you have arthritis or not. Maybe you could try seeing a Rheumatoid specialist at some point (they may require proof of arthritis so at least see your GP about it).
