Colonoscopy report please help me understanding the results

Hi Folks! Just a quick history of mine: i have been suffering from persistent abdominal pain (burning type of pain) for the past two to three years and since then i have had frequent bowel moments (usually take Imodium once or twice a week to hold my BMs).I have been to numerous GI specialists and almost all of them diagnosed me with IBS-D . I have undergone numerous tests in the past four to 5 months i.e Stool Occult blood( Normal) Celiac disease test (Normal), H.pylori test (Normal),stool culture MacConkey agar ( Normal), CT scan abdomen and Pelvis with contrast (Normal) , Thyroid Profile test (Normal) ,FreeT4(Normal) Calprotectin test (Normal) and numerous other tests which came back normal.. I recently had a colonoscopy.. The GI doctor found nothing while doing the procedure but my biopsy report came back as " MILD CHRONIC INFLAMMATION OF THE TERMINAL ILEUM". I will see my G.I doctor after a week .What does my biopsy result mean? Is it conclusive of IBD? please give me your opinion. And secondly i am just fed of this persistent pain in my abdomen and loose stools (been on numerous antibiotics as well and some of them usually did make my condition even worst). Thanks
 
Do you have a follow up with your doctor to discuss the biopsy results? I won't say anything conclusively because I am not an expert but it sounds like you could have IBD. Let us know. Best to you.
 
These are the "borderline" cases that make Crohn' such a challenging disease. To be honest, this is not an exact science, and as far as I understand, there can be other reasons for the inflammation. Having said that, the TI is a commonly affected part in Crohn's, but I think that is the case in TB as well (not sure about this fact). This is not conclusive, to answer your question. Your FC is normal; can you post the results of your blood tests? However, none of these are confirmatory.
 
Well i have seen my GI recently and he said that it's normal and it all lies inside my head but i know there is something wrong going inside me because i usually have persistent burning type of abdominal pain and cramps are usually painful proceeded by loose stools i am just wondering to see another GI because i just want to live a normal life.
 
It would probably help to know the exact values of the FC and the bloods. In any case, the GI has to explain the inflammation, mild or not. IMHO, otherwise he is not your or the taxpayers' money's worth.

I would wait to see what your Gi says given your current biopsy results
Since your Gi knows your personal history and not just one single blood test or FC
Inflammation can be caused by NSAID use , tb , infection , bechets, crohns and a few other things .
The key is figuring out the why .
I know it stinks when your not dx but getting the right dx is important since the wrong drugs won't be helpful.

Microscopy:
Site – Terminal ileum mucosa
Inflammation – Mild chronic inflammation
Granuloma – Not seen
Parasite – Not seen
Dysplasia – Nil
Neoplasia - Not seen

Opinion: TERMINAL ILEAL MUCOSAL BIOPSY - MILD CHRONIC INFLAMMATION
 
Give above is my Colonoscopy Biopsy Report: Thanks all of you for your replies. I haven't used NSAID much and if i remember i used them long time ago and for not more than 5 to six days

Some pathologist require non caseating granulomas to be found in order to check the crohns box on the pathology report
But only 30% of those with crohns have granulomas found on biopsy

Taken by themselves, your symptoms sound like IBS, but the inflammation suggests mild IBD. One possibility is that you have two things going on at the same time, say a very mild case of Crohn's, or some non-specific ileitis, plus an IBS overlay.

Did the unsuccessful antibiotics you have tried include Rifaximin?
 
Hi Scipio Thanks for your reply. yes i have tried rifaximin many times and it does work but after sometime i observe it loses its efficacy and the last time i saw my GI ( a week ago) she told me to take rifaximin 550 mg once a day for indefinite time i asked her its an antibiotic. should i take it for long term? she said don't worry you can take it because it is a non-absorbed antibiotic. She also gave me a folic acid tablet once a day for 15 days and also an antispasmodic three times a day for 15 days. The problem is usually the abdominal cramps and burning sensation pain in my abdomen (sometimes i can easily feel that there are some wounds inside my belly) followed by loose stools and the color of loose stools are usually green to dark green. I do wonder now. will I be able to live a normal life? free of abdominal pain, loose stools and food intolerance because it has effected my life alot. imagine when you are invited by your friends/relatives for a supper/lunch/dinner.? And due to your conditions you can't enjoy their company and of-course the meal as well. i just want the doctor to at-least properly diagnose the disease because it is pretty much clear that i have been suffering from this problem/disease for the past 2.5 to 3.5 years. I have found that even the Antispasmodic doesn't touch my abdominal pain.. I am also thinking to see another GI in our town because i have seen his qualification on the website and it is mentioned that he is a gastroenterologist and hepatologist and also a bowel cancer screening specialist.

Does the abdominal pain go away or lessen after you pass the stools? If the answer is yes, that would be one more piece that suggests IBS. But it would not rule out also having IBD.

I think some of the confusion is having IBS doesn't mean something isn't wrong
It just means that whatever is causing the problem and making you feel horrid is not doing damage to your cells in your intestines

Somethings that can help
Have you tried a tens unit?
It can stop pain ( lessen it ) from visceral sensitivity ( meaning your nerves got worked up over something at some point but haven't stopped sending out pain signals even though there is no reason to )
Tens units needs to be used for a few weeks to see a massive difference
Have you tried formula only ( ensure /boost etc) for few days ?
Ds uses this as a mini reset
EEN is used to induce remission in Ibd patients but typically is given for 6-8 weeks
Ds has done this twice

While on formula reintroducing foods one at a time
Each new item for three days to see what made Ds worse
In his case chicken beef wheat or anything with fiber


There is the fodmaps diet which is similar

Not saying you don't have Ibd but sometimes even Ibd guts are more sensitive to things including food

I have IBD and my cramps and pains always stop after a bowel movement. I have most experience with disease in left colon and rectum but I have already had an episode in ileum and only got cramps with diarrhea after eating at some restaurants. I have read all different types of pain patterns in the forum.

Tazz1985, I also have a dx of IBS. I have chronic inflammation of the stomach. All other tests are normal.

I first tried the FODMAP diet when my daughter was very ill. I noticed an improvement with in days. My daughter noticed no change and was later dx with Crohn's disease. My trigger appear to be wheat.

The drugs used to threat Crohn's all have serious side effects.

Hi,Tazz,,that's how my ordeal started..I would suggest a small bowel through with a pill camera.Go to a digestive disease center and have them do a colonoscopy..Sounds like Crohn's
 
Hi Tazz. "Mild chronic inflammation of the terminal ileum" is not necessarily Crohn's, but I think it is a indication. There are other conditions that can cause inflammation in the T.I.. I think you should get a second opinion. I was diagnosed for 6 years with IBS. It was wrong. I personally think anybody who gets diagnosed with IBS should get a second opinion, just to make sure.

I agree. Last year, along with ,Crohn's, I was told I had IBS. Later in the year, after still experiencing symptoms, I was tested again and it was actually the Crohn's.
 
I would suggest to NOT use NSAIDS at all. The tablets can get held up at the Terminal Ileum and erode it, causing damage and inflammation.

I believe that is part of what caused my UC and Terminal Ileitis.
