Transferrin Saturation - TS

Transferrin Saturation (TS) is obtained by the formula: (serum iron 100)/TIBC, where TIBC is the marker for circulating transferrin. Iron deficiency is associated with TS of less than 18% and normal TS is 25% to 50%.

In anemia of chronic disease, iron is sequestered from circulation while transferrin levels drop, causing a near-normal transferrin saturation level. Thus, underlying iron deficiency can be present in anemic patients with normal TS who have an underlying inflammatory disease process such as Crohn's Disease and other forms of IBD.
